Post-acute care interventions
A successful transition from the hospital to post-acute care settings can improve patient outcomes. Leland and colleagues (2019) conducted a scoping review to identify community transition interventions. Studies included in the review focused on transitions from institutional settings to the community and included hospital readmissions as an outcome.
