AOTA Conflict of Interest form
All AOTA volunteers must complete a Conflict of Interest form at the start of each year (July 1) of their term on an AOTA volunteer group, even if they have no conflicts to disclose. Interim changes to one’s conflict status must be reported by submitting a NEW form and notifying the volunteer group’s chairperson.

Instructions for accessing the AOTA Conflict of Interest form
After clicking the Conflict of Interest Form link below
- Login with your AOTA username and password.
- Select the CONFLICT OF INTEREST FORM tab in the navigation bar, or scroll to Step 3.
- Select Click here to begin a new submission.
- The COI form will appear.
