Preparing for discharge home after acute hospitalization is a critical phase of rehabilitation. Major medical events and ensuing disability have long-term impacts on entire families. Clients’ and caregivers’ nuanced and variable needs can contribute to their feeling ill prepared for returning home. One factor known to detrimentally impact this transition is poor quality preparatory education and training (Denham et al., 2019).
