Mental health needs of transgender & gender diverse individuals

Transgender and gender diverse (TGD) identity is not a mental illness and treating it as such can lead to complex trauma, mental health conditions, and increased suicidality (Coleman et al., 2022).

Research states that affirmation and being able to live as one’s authentic gender positively impacts the mental health and overall well-being of:

  • transgender children (Ehrensaft, 2016; Olson et al., 2016)
  • teens (Simons et al., 2013; Tishelman et al., 2015)
  • adults (Bauer et al., 2015; Heylens et al., 2014)

This topic provides a general overview of the various needs of transgender and gender diverse individuals. It not comprehensive or all-encompassing and encourages further discussion and discussion.

Mental health factors

Mental health conditions are not solely due to an individual’s TGD status but may be due to stigma, discrimination, marginalization, violence, complex trauma, and societal attitudes (Coleman et al., 2022).

Overall, TGD individuals:

  • Have a significantly higher risk of suicidal behaviors or ideation and are more likely to suffer from depression and anxiety when compared to the general population.
  • Are at a higher risk of substance use disorders (Hughto et al., 2021; James et al., 2016).
  • May develop post-traumatic stress disorder due to trans-specific discrimination and victimization in various social contexts (school, family, community, work, etc.) (Austin & Craig, 2015).

A significant source of discrimination is within healthcare systems where TGD individuals experience negative interactions with their healthcare providers (Bauer et al., 2009; Howard et al., 2019). This document aims to highlight different factors and issues that can negatively impact a TGD individual's mental health as well as how occupational therapy practitioners (OTPs) can provide support.

OTPs can advocate for and support participation in meaningful occupations for TGD individuals by promoting client and occupation-based interventions that assist with managing their mental health (American Occupational Therapy Association [AOTA], 2017).

OTPs should also be aware of the increased risk of mental health conditions in TGD individuals. OTPs have the skills necessary to:

  • Perform comprehensive mental health assessments.
  • Prevent and/or treat mental health conditions through occupational performance.
